Submit a Youtube performance video here to enter the Live Arts / My Eye Xpert Scholarship Contest.

1. The video should be less than 5 minutes in length
2. There will be a  TOP 3  in each Performance category (Acting, Instrumental, Vocal)
3. The 3 Videos in each category will be pre-selected by a panel of judges  
4. Top 3 will be announced on stage the night of the show!
5. A public vote will be conducted online (My Eye Expert Webpage)

*Overall winner receives a $1000 scholarship sponsored by My Eye Xpert Gurnee
*Each category vote leader wins a pair of designer sunglasses from My Eye Xpert, Gurnee
