Membership Application Form

Choir: $5 per month; $360 per year (Non-member Annual Fee)

DanceSport: $18 per half year (Joined after June); $36 per year (joined between Jan and June)

DramaWorks: $5 per month

Toastmasters Club: $40 (One-time registration fee); $20 per month

I understand that there will be no refund of the membership fee on a pro-rated basis for my withdrawal in the course of the year. I also understand that my membership will be automatically renewed and the membership fee shall be debited to my account at the beginning of each period as indicated above (monthly, quarterly or yearly), unless I withdraw from the section in writing by the end of the year.
