Please list the class(es) you wish to enroll in including the CLASS TITLE, DAY, & TIME - Please select the class levels based on what you have been taking this past dance season (Fall 2021 - Spring 2022) or ask your teacher before enrolling! If you are not a current Unity student but do have previous dance experience please detail your experience below. We will reach out with class recommendations! *
Dancewear Notice | All dress code items can be purchased through our UDS dancewear services. Please stop by the office to be sized and to purchase dress code items!
Upon submission of this form a UDS representative will be in contact with you to confirm your schedule. You will also receive an invoice for your Summer Tuition payment. Submission of this form does not guarantee a spot in your selected classes. Summer Tuition Fee must be paid and confirmation from the UDS office must be obtained to guarantee your spot in class.
If you do not currently have a card associated with your account, we highly recommend uploading your billing information in order to allow touchless payment processing. You can add your billing information by logging into your MindBody account online.
