In Consideration of my participation in the Florida United Lacrosse, LLC sponsored events and activities I, the undersigned, on behalf of the participant, my spouse, family members, heirs, executors, administrators, representatives and any other third parties entitled to assert any rights by or on behalf of the Participant, for good and valuable consideration, intending to be legally bound, hereby agree as follows: 1. Released Parties: I am signing this Agreement for the benefit of the organizers and directors of Florida United Lacrosse LLC., Club coaches, officials, supervisors, trainers, spectators, volunteers, vendors and spouses, and the respective trustees, directors, officers, members, managers, agents, trainers, volunteers, staff, employees, heirs, executors and assigns, of any and all of the foregoing. 2. Waiver and Release: I am fully aware of the risks related to Participants voluntary participation with Florida United Lacrosse LLC. These risks are often unpredictable but inherent in the game of lacrosse, a full contact sport. Risks include, but are not limited to, serious physical and/or emotional injury including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in a lacrosse event and related sports conditioning activities. I further agree on behalf of myself, my heirs, and personal representatives that Florida United Lacrosse, LLC., along with the Released Parties listed in paragraph 1 above, shall not be liable for any injury, loss or damage occurring as a result of my participation with Florida United Lacrosse, LLC. I knowingly and willingly assume all such risk as well as responsibility for all personal and/or economic injuries and losses which Participant might sustain in connection with Florida United Lacrosse, LLC and hereby consent to Participants participation despite the risks. 3. Medical Attention: I hereby give my consent to Florida United Lacrosse, LLC. to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and urgent or emergency medical services as warranted in the course of my son’s participation with Florida United Lacrosse, LLC. sponsored or sanctioned Events. 4. Readiness to Compete: I will only allow Participant in those competitions or activities in which I believe he is physically and psychologically prepared to participate. I certify that Participant is in good health and has no health-related condition that would make his participation with Florida United Lacrosse, LLC. hazardous to him or others. 5. Release and Consent: I fully and forever give up, release and discharge the Released Parties from and against all claims and causes of action for Injuries and Losses which Participant (or any Releasor) may have in any way as a result of Participants participation with Florida United Lacrosse, LLC. I consent to have pictures, video footage and other similar media taken of the participant and agree that such media is the property of Florida United Lacrosse, LLC and may be used to their discretion as needed. I understand Florida United Lacrosse, LLC may be revoked membership at any time by the Club Directors and that no refund will be given. As Parent, or legal guardian of this Participant, I hereby verify by my that I have read and fully understand each of the conditions under the Participant Wavier & Release section above for permitting my child to participate in all Florida United Lacrosse, LLC sponsored events and activities, and I accept each of the conditions. *
