Potential Player Conflicts Millers Tryouts
We ask that you make every attempt to attend tryouts on the predetermined dates, however we understand there are some circumstances that may prevent this from happening.  If your player has a conflict for either date, please provide all requested information.  If your player has conflicts both tryout dates, please complete this form twice, once for each date.
Our registrar will be in touch prior to tryouts to confirm absence and discuss potential makeup dates.
