This form is for seniors only. Once you decide what College/University you will be attending, they will need a final transcript that reflects all of your high school coursework. These transcripts will be sent in June. Please complete the request below to have your final transcript sent.

If your transcript is being sent to an Out-of-State school, you will be required to pay a $3.00 fee to Ms. Dixon in the front office. Your transcript will not be sent until this fee has been paid.

If you are attending an In-State-School then there is no fee to have your final transcript sent.
