CHAP Mentor Application
This form is to be completed by homeschool parents who desire to serve other homeschoolers by being a mentor to them. If interested, please complete the following steps:

1. Watch the mentor training video at chaponline.com/mentors
2. Complete this form.

Note that after submitting the form, a mentor leader will be in touch to chat with you. Thank you for supporting homeschool families!

Next, please read CHAP's statement of faith and answer the following questions (below):
   > The Bible is the inspired and infallible Word of God and constitutes His completed and final revelation to man. The Bible, in its original autographs, is without error in whole and in part.
   > God has existed from all eternity in three persons: God the Father, God the Son, and God the Holy Spirit.
   > Jesus Christ was God in human flesh, being fully God and fully man, except without sin.
   > All men are in violation of God’s righteous requirements and His holy character both by nature and act, and are therefore under His wrath and just condemnation.
   > The central purpose of the coming of Jesus Christ was to pay the penalty for man’s sin through His substitutionary atoning death on the cross, the successful accomplishment of which was attested to by His subsequent visible, bodily resurrection.
   > Salvation is offered as a gift, free to the sinner. This gift must be responded to by individual faith, not trusting in any personal works whatsoever, but in the sacrificial death of Jesus Christ alone.
   > All human life is sacred and created by God in His image.  We are therefore called to defend, protect, and value all human life.
   > God wonderfully and immutably creates each person as male or female.
   > The term “marriage” has only one meaning: the uniting of one man and one woman in a single, exclusive union, as delineated in Scripture.  God has commanded that no intimate sexual activity be engaged in outside of a marriage between a man and a woman.

That concludes the questionnaire portion of the application. The following two paragraphs show what a mentee will see on the main search page when looking for a mentor:

"Hello! Welcome to the CHAP Mentor Network. As you search for a mentor to connect with in your area, keep in mind that our network functions similarly to our support group and evaluator searches. All CHAP mentors are listed here so you can see who lives in your area, but it is the mentor's responsibility to maintain a schedule that works for their lifestyle. CHAP does not match mentor to mentee. The mentor network functions more like the evaluator search where you look for someone and then reach out to them to chat about availability. It could be that the mentor you choose to contact first will have a full schedule and cannot take on another mentee. Do not despair! Try another mentor. It could be that you end up connecting with someone out of your immediate area.

We pray that this network of homeschoolers will bless you and encourage you on your homeschooling journey. And if you have experience and support to share with others, please consider being a mentor, too!"
