Toonspeak Child Welfare Concern Form
A report should be made by the person who has had the initial concerns about a child’s welfare using this form.
Your duty is simply to report concerns. It is not your responsibility to try and fix issues.
If the child is perceived to be in immediate danger, the Toonspeak team member working with the young person should contact the appropriate Social Work dept/ police, and also complete this form.
The form should be filled in even if there is a concern rather than an actionable & immediate risk to safety. The purpose of this form is to record your concern, and ensure information can reach the people with the expertise to deal with Child Protection issues, rather than make any investigation or judgement call pertaining to a child or young person’s situation (unless in clear and immediate danger).
Avoid asking leading questions or attempting to investigate, this is a record of observation and/or disclosed information.
