Bands of Dreher Health and Wellness Packet 

Please fill out the following health forms required for participation in the Bands of Dreher (“BoD”): 

  • Richland School District One Sports Health Form (*Please note that the term, “student athlete” also refers to “student musicians” )

  • BoD Health History and Medical Evaluation Form 

  • BoD Informed Consent, Release of Liability, and Assumption of Risk Form 

 

Please be aware that all bands/ensembles have the possibility of travel, light to moderate exercise, and performance in hot weather conditions, and that injuries and medical emergencies are possible during practices, performances, and related travel. 

 

In any paperwork that specifies “coaches/athletic trainers/etc.”, please note that this includes the Band Director, Assistant Director, and Staff, as well as qualified adult volunteers present at any activities.  

 

The BoD does not require students to submit to an official physical examination prior to participation, but we do require that all paperwork is filled out completely by the parent/legal guardian of any students under the age of 18 *before* participating in band-related activities.
