All players and parents are required to provide registration information to OJCUP for administration purposes. This google form will be used in place of paper registration forms, and contains the following sections:
a)     Player Information;
b)     Player Tournament Availability;
c)     Parent / Guardian Information; and
d)     Parent / Guardian Tournament Attendance
e)     Medical Questionnaire for away tournaments
