Legal ID Changes Financial Assistance Form
This form is to be used to request financial assistance for legal ID changes, including fees to change your birth certificate, driver's license or DMV identification card (Real ID available), passport, and court order affidavit. It is for trans and gender diverse Oregon residents with financial need.

Please fill this out to the best of your ability and a team member will follow up with you as soon as they are able, in order to gather more information and schedule a meeting with you. If you are issued financial assistance and do not use it within 30 days, you must wait six months to reapply to the program.

Note on Passports:
We are currently not providing financial assistance to updates to the gender marker on a US passport. This is due to the changing federal policy and rising evidence that the US Department of State is withholding applications and supporting documents for requests to amend the gender marker. 

There are still a lot of unknown factors (i.e. what if it's your first passport, what if all your supporting documentation includes your new name and gender marker, what if you're renewing a passport that has already been amended, what if you're just amending your name, etc.). We are continuing to monitor the situation and will keep you updated as it develops. 

If you are still interested in applying for an amended passport, please select it from the options on the second page of this form. We will reach out to help you determine the best course of action for your situation.
