Please complete this form if you require First Aid supplies. DCU Sports Clubs First Aid Kits should be stocked to cater for up to 25 people as per the Health and Safety Authority (HSA) recommendations. Please see the list below for the contents that your first aid kit should include. This stock is the minimum legal requirement for a First aid Box. It is the clubs responsibility to replenish first aid stock as necessary.

Please can you do a stock take of your First Aid kits and if you require supplies please complete the order form by Tuesday the 12th of October 2019.
