2023 Regulation Waiver Request Form

Before completing this form Captains should review and be familiar with National and PNW regulation 1.02A Waiver of Regulations and Procedures Prohibited.

1.02A Waiver of Regulations and Procedures Prohibited. Except where a waiver is specifically permitted by a USTA League Regulation or by a USTA League Procedure, no USTA League Regulation or USTA League Procedure shall be waived by any Sectional, District/Area or Local Association or by any Sectional, District/Area or Local League Coordinator. Any violation of this Regulation shall be subject to such penalties as may be imposed by the USTA League Committee. Such penalties are not subject to appeal.

1.02A1 (PNW REG) – A waiver may be submitted for any USTA/PNW Regulation. Waivers will be considered based on the hardship of the specific situation, alternative options which have been tried or failed, and/or how the waiver will benefit the Local League or PNW Section.

Your request will be reviewed by USTA/PNW Managing Director of Provider Services Elece Fiocchi before being forwarded to the Adult Competition Committee for consideration.
