A) Liability:
As a participant or parent of a student at River City Dance & Performing Arts Theatre, I understand that:

i. Participating in dance or any other exercise activity involves risk of injury.
ii. RCD&PAT are not liable for injuries sustained from any activities during class, session, performances or any other related events.
iii. The theatre is not liable for any personal belongings lost, stolen, or damaged.

B) Medical Release & Emergencies:
As a participant or parent of a student at River City Dance & Performing Arts Theatre:
i. I grant permission to authorize any emergency medical treatment that may be required for self and /or child.
ii. I confirm that a medical evaluation has been performed and student’s registration is cleared to enroll in an exercise program.
iii. I have informed RCD&PAT of all medical conditions that may limit participation in class including food allergies.
iv. I understand that the staff is not responsible for diagnosing or treating any diseases or disabilities.

C) Dance as a Contact Sport:
I understand that:
i. Dance is a contact sport of the arts.
ii. Teaching dance is a hands-on profession when developing a student's technique.

D) Media Waiver:
I understand that:
i. RCD&PAT has permission to use students' likeness in promotional photographs and videos without payment or any other consideration.
ii. All materials will be the property of River City Dance & Performing Arts Theatre.
iii. I understand that I will be receiving periodical emails and mailings from River City Dance & Performing Arts Theatre, and will call or email if I wish to be removed.

E) Code of Conduct and Handbook:
I understand that:
i. Both, students and parents will conduct themselves respectfully, without malice towards the staff, students, or property of RCD&PAT at all times and understanding that failure to do so may result in legal action and will absorb all expenses including legal associated.
ii. River City Dance & Performing Arts Theatre reserves the right to suspend or expel any student if behavior, whether physical or verbal, causes extreme hardship to any person(s) or property listed.
iii. I have read the Family Handbook and agree to all terms and conditions listed.
