I know that participating in a road race, cross-country race or track race is a potentially hazardous activity. I should not enter and run/walk unless I am medically able to properly train. I agree and abide by any decision of a race official relative to my ability to safely complete the event. I assume all risk associated with participating in the event including, but not limited to falls, contact with other participants, the effect of weather, including frigid waters, shrinkage, traffic and conditions of the road, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and inconsideration of acceptance of my entry, I, for myself and anyone entitled to act on my behalf waiver and release Club Northwest, USA Track and Field, all sponsors, their representatives and successors from all claims or liability of any kind arising out of my participation even though said liability my arise out of the negligence or carelessness on the part of the persons named in this waiver. I grant permission for all the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose. This waiver may also be used to certify that my child has permission to compete in this event, is in good physical condition and that event officials my authorize necessary emergency treatment. You must be 18 years of age to legally complete this waiver. If the registrant is under 18, an authorized adult must complete this form.
