EDS 128A is reserved for students in their final year who have met the prerequisite course requirement and intend to become elementary school teachers.

Do NOT enroll in any EDS practicum course if you have previously failed an EDS practicum course, have been convicted of a felony, or are required to register as a sex offender.

If you have previously withdrawn from an EDS practicum course, please consult with the program director, Dr. Cheryl Forbes (cforbes@ucsd.edu), before completing this form.

Course requirements:
- Completion of Prerequisite: EDS 130, 131, 133, 135, 136, 137, 145 or 146
- Negative TB Test taken within the past 60 days brought to the first day of class
- 40 hours of fieldwork over the course of the quarter
- A reliable method of transportation to and from the school site (up to 30 miles from UC San Diego)

Concurrent enrollment into EDS 139 is required. Please choose the EDS 139 section with instructor Sharon Fargason (sgfargas@ucsd.edu).
