If you're gifting a single donation's rewards to multiple players you MUST fill out this form separately for each individual player with their individual amounts. For example, if you donate £15 and want to give £5 to three separate players, you need to fill out this form THREE times with £5 stated each time.
This form is only to be filled out by the person that made the donation. If someone is gifting donator to you, THEY must fill out this form, not you.
If you set up a recurring donation, you MUST fill this form out each month after the payment is sent, or it will not be verified.
