OFS Legacy Application
Dear Applicant:

We are still accepting applications during the COVID-19 closing. When you apply, call to set up a phone interview. If you are school-aged, you should email or call your counselor to let them know you have applied.

Read below for details:

Thank you for your interest in the Operation Fresh Start Legacy Program. If you are between the ages of 16 and 24, want to work on your high school diploma (or equivalent), and want to make major changes in your life, then complete the following application.

NOTE: If you are under age 18, you must be enrolled in your school district and talk to the guidance counselor at the high school in your district to get a referral for the Operation Fresh Start Legacy Program. Applicants under 18 must be either in their 4th year of high school or the 4th quarter of their 3rd year.

An applicant MUST be a citizen, national or lawful permanent resident of the United States.

IMPORTANT: To apply you must complete the following steps:

1.) Complete ALL parts of this application

2.) On the day after submitting your application, call Bill at (608) 212-8728 or Greg at (608) 576-9068 to request an Initial Interview.

3.) All applicants must bring the following documents to be eligible for the program:

     A.) Official Birth Certificate--it must be a certified copy. Copies, birth registrations, and hospital certificates are not acceptable.
     B.) Valid Government-issued ID--Driver's License or State ID accepted.
     C.) Government-issued Social Security Card--copies are not accepted.

4.) If you are accepted into the program, you will be required to complete an income verification. Some restrictions apply.
