Mommy & Me Dance Classes
This 45 minute class is structured to stimulate coordination skills, listening skills, balance, music interpretation, creative movement and motor development in the youngest dancers, ages 1-6 years. This program is an excellent introduction to the world of dance and lots of fun for Mom and dancer!*Mom will also participate in class with the dancer!* Mommy & Me class does not participate in the recital.

Class Info:
Please arrive 10-15 minutes early. Bring water and a light towel. Wear comfortable clothing. Athletic sneakers, dance slippers, or barefoot are acceptable.

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance/fitness. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to the Concrete Rose Dance Academy, it’ officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “The Concrete Rose Dance Academy”.

I hereby agree to release Concrete Rose Dance Academy and hold Concrete Rose Dance Academy harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance/fitness on behalf of the participant.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I also give The Concrete Rose Dance Academy permission to use my picture in or on any form of advertisement for The Concrete Rose Dance Academy or a Concrete Rose Dance Academy affiliated event.

If I am a minor, my parent and / or legal guardian has also signed this document releasing The Concrete Rose Dance Academy from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in The Concrete Rose Dance Academy. I warrant the below information is complete and correct.

I further release The Concrete Rose Dance Academy of all liabilities associated with my attendance at The Concrete Rose Dance Academy.

I acknowledge that I have read and understood this waiver in its entirety.
