This form details the 2023-2024 School Field Trip Permission, Waiver, Release and Indemnity Agreement. This agreement covers all travel associated with school day field trips and school-related activities and events, including but not limited to offsite P.E. classes (at MLK Park), local day trips (eg.—Coyote Point, Central Park), and offsite experiential trips (eg.—Academy of Sciences, San Jose Tech Museum). Please note that families will be required to complete a separate permission form and waiver agreement for overnight trips.

THIS IS A REQUIRED FORM FOR STANBRIDGE FAMILIES TO COMPLETE FOR TRAVELING OFF CAMPUS FOR ANY SCHOOL-RELATED ACTIVITIES.
