After submitting, you will receive a copy of this form in your email for your records.

Upon receiving this form, HR will process eligibility approval within 5 business days and notify the Financial Aid Office for further processing. Employee submitting form will also be copied on this email.

NOTE: First time applicants enrolled full-time must file the Free Application for Federal Student Aid (online at www.fafsa.gov) BEFORE a Tuition Waiver will be processed. Questions on the FAFSA? Contact Financial Aid.
