MHRR believes strongly in education & discussion rather than discrimination. Certain household pets or lifestyles do not automatically rule out a home for adoption. Please be honest and detailed when answering this questionnaire. This helps us determine which pets would be most ideal for your home and to make sure both you and your pet are happy for a lifetime.

We now require a deposit for borrowing supplies. If they are not returned within 2 months the deposit will be deemed non refundable. If you require an extension you must let us know. This deposit is in addition to the adoption fee. Selecting this lets us know that you have read and understand this. *

If this pet gets sick during the foster-to-adopt period, we ask that you bring the pet to Azalea Lakes Veterinary Clinic in Baton Rouge or Avian & Exotic Animal Hospital of Louisiana in Metairie. Are you willing to do this? *
After adoption, which veterinarian will the pet see for annual checkups and illness/injury? *

With the foster-to-adopt process, you are asked to call or email with a report of how things are going at least once weekly until the adoption has been finalized or the pet is brought back.  Are you willing to do this? *
