Please list names, grades, and email addresses of all students who worked on this project so we can be sure to reach you with the results. Be sure to give us email addresses that you will check in August. School email servers usually have tight security and reject our emails, so if you can give us a personal email address that would be great, but please check with your parents. If not, we will contact your teacher or club/youth group facilitator. *

A goal of this project is to educate people in your community about how they can help address the problem you researched. You must also present to people outside of your school in order for your work to have the most impact. Please tell us who you presented your work to—this can be people in your faith community, local community council meetings, or any public venue. Your teacher, parent, or guardian must send an email confirming that you presented your project, who you presented to, and the date of your presentation to granite@centropa.org. *
Anything else you'd like to tell us as you submit your project for the Milton Wolf Prize.
A copy of your responses will be emailed to the address you provided.
