Eligibility: Children who completed Kindergarten thru 5th grade in the previous school year (2021-22) may be registered for enrollment in the program at any time.

Enrollment Agreement
Please read the following statements. In order to complete the enrollment, you must select 'yes' beside each of the following statements.
I understand the Program Staff will assume full responsibility for my child from the time he/she arrives at the Program until my child leaves the Program according to the written instructions for departure. *
I understand if a medical emergency arises, the Program Staff will attempt to contact me. If I cannot be reached, the staff will contact my child’s doctor. If the emergency is such that immediate hospital attention is necessary, an ambulance or emergency vehicle may take my child to the hospital. *
I understand the Summer Fun Program is continually covered in prayer by our community churches. In addition, your child’s name, age and picture may be given to a Christian Adult who will pray for them individually throughout the year. *
My child has permission to attend field trips the Program will take throughout the summer. I understand that a church bus, and, if needed, separate vehicles will be provided for transportation. *
I agree to grant Common Ground Ministries the right and permission to publish images/pictures of my child for PROMOTIONAL PURPOSES of Summer Fun, in which they may be included in whole or in part. I understand that my child's name will not be used. *
I agree to adhere to the stated policies and procedures of the Summer Fun Program as stated here, and give my child permission to participate fully in this program. *
