Survey of Interest for Early Intervention Evaluations
Effective for referrals made to early intervention May 1, 2022 and ongoing, the Colorado Department of Human Services (CDHS) Early Intervention (EI) Colorado will be assuming the responsibility for performing early intervention evaluations to determine eligibility for services under Part C of the Individuals with Disabilities Education Act. These evaluations will be completed in-person, virtually, or using a hybrid process (1 evaluator in person, 1 participating virtually) in a culturally competent manner utilizing a state-approved evaluation tool or tools. Entities who provide evaluations will not be excluded from providing direct early intervention services.

This survey is being conducted only to assess the statewide level of interest in applying for an upcoming Request for Proposal (RFP) that will be open to all interested parties in the Fall of 2021.  This survey should not be misconstrued as being part of the CDHS procurement process, therefore survey responses will be anonymous. This survey will be open until 5:00pm on October 31st.
