9th Grade Lagoon Participation Parent Consent Form
This form gives parental consent for student participation in OCJH 9th Grade Lagoon Day on Wednesday, May 24th. Students who have failing grades or are under disciplinary action will not be allowed to participate. It is up to the parent and student to monitor their grades to qualify for this activity. There will be a student review before getting on buses that morning. THERE WILL BE NO REFUNDS.

Tickets are $54.00 at the OCJH's Finance Window,   or you are able to pay at  myschoolfees.com   .
Parents are responsible for the students transportation to and  from the Trax station.
 Students will be meeting Chaperones in the parking lot at the Orem Central Trax station (1350 West 900 South)at 6:50 am(Parents will not be allowed to drive students up to Lagoon). The students will be responsible for providing or buying their own lunch.

Tickets will NOT be sold without verification this form has been filled out, submitted, and received by OCJH's Finance Office.

I understand that to be guaranteed a ticket, it needs to be purchased by Tuesday , May16th. PARK TICKETS WILL NOT BE SOLD AT LAGOON'S GATE, and must be purchased at Oak Canyon. Students who have a Lagoon Season Pass will need to contact the front offices to let them know. and then present their season pass at the gate.  Unless special arrangements have been made with administration,

I understand  at the end of the day my student will need to report to the chaperones by 3:30 pm in the parking lot at the front entrance of Lagoon. They will need enough time to shuttle back to the Front runner to catch the 4:39 departing train. They will arrive back to the Central Orem Station  at  6:03 pm.
I understand that I am responsible for transportation to and from  Trax  ( If your student , fails to meet their chaperone at the end of the day it will be the responsibility of the parent/guardian  to pick up your student from Lagoon.)
