Complete this form for your students. Please note that payment for both coaches and students must be brought with you to camp. The cost is $30 per student and $25 per coach. Make checks payable to AGQBA. Your receipt of this form will serve as your invoice.
Although you are selecting a day to attend camp, please understand that this may need to be moved. You will be notified by Nov. 6 if your selected date is not available. Please note that your team will only participate in one day of camp. You cannot have students attend both days.
Camp is limited to no more than 50 students each day. There must be a minimum of 35 students registered to have camp each day.
