Camden Middle School Field Study Refund Handling Form
Dear students and parents/guardians,

Many of our Field Trips for this year were cancelled due to COVID-19. We have three (3) choices in which the money can be handled. NOTE: A transfer of funds will be the most expeditious means of handling these funds. Any refund processed may take several weeks due to school and district closings for COVID-19. Please choose which way you would like to receive a refund. Please note, if your student owes any fines on textbooks, library items, Chromebook/chargers, has a negative lunch account, etc. these will be paid before the refund is given.

***All money put on my student’s lunch account.

***Apply money toward any outstanding debt including lunch account, library fees, technology debt, textbooks, and applying any left to my student's lunch account.

***A refund in the form of a check from the Kershaw County School District Office pending verification and clearing of any outstanding debt to CMS and/or Kershaw County School District.

(Please keep in mind that if your student owes any fines on textbooks, library items, Chromebook/chargers, has a negative lunch account, etc. these will be paid before any handling of funds.)

Any questions or concerns will need to be addressed through your child's homeroom teacher or CMS.
