Audition Application
Audition clinics and auditions for all Honors Dance Company groups are May 4 & 6.  All candidates are required to attend the  clinic(s) for the group(s) they want to audition for to learn the audition material and pay the $20 fee for each clinic attended.  Please bring $20 cash or check (made payable to Mater Dei High School) on the day of the clinic.

Hip Hop Audition Clinic - Saturday, May 4, 9-11am

Dance Conservatory Audition Clinic - Saturday, May 4. 11:30am-1:30pm

Song Audition Clinic - Saturday, May 4. 2:00-4:00pm

AUDITIONS (all) - Monday, May 6, 3:30-6:30pm

  • Hip Hop - Check in 3pm, 3:30-4:30
  • Dance Conservatory - Check in 4pm, 4:30-5:30
  • Song - Check in 5pm, 5:30-6:30

Location: All clinics and auditions will be held in the Monarch Pavilion at Mater Dei High school.  Signs will be posted to direct you to the building.  Please plan to arrive a few minutes early to check in.

Dress Code: The below dress code is required for both the clinic and for auditions.  Note that if you are auditioning for multiple groups, you will need to bring multiple outfits to dress appropriately for each audition.  All dress code should be Catholic school appropriate (no midriffs please and if wearing shorts, please make sure they are not too short)
  • Hip Hop - Footwear: sneakers, Attire:  loose fitting sweats/joggers, t-shirt, tank top, or sweatshirt.  Leggings are also welcome. Hair should be neatly pulled out of their face.
  • Dance Conservatory - Footwear: half soles, jazz shoes, or ballet shoes, no socks please (our Marley floor is slippery!) Attire: leggings, shorts or joggers you can move in, tank top or leotards welcome.  Colors are encouraged so that dancers stand out and show off their individual style. Hair should be neatly pulled out of the face in a ponytail or bun.
  • Song - Footwear: jazz shoes or half soles, Attire: ALL BLACK fitted dance wear including leggings, shorts, tank tops or fitted t-shirt. If you have poms, please bring them.  Poms will be provided for dancers who do not have their own.  Hair should be in a clean low pony.

ASSUMPTION OF RISK, WAIVER, RELEASE, AND HOLD HARMLESS  
I am the parent and/or legally authorized guardian of the child participating in the Dance Department Audition (the “Audition”). On behalf of myself, my spouse, my child/ward (the “Participant”), and my/our heirs, personal representatives, and assigns, I understand and agree to the following: 1. Assumption of Risks: that participation in the Audition and related travel, events and activities is entirely voluntary and that participation in the Audition involves risks of injury due to certain inherent dangers that cannot be eliminated regardless of the care taken to avoid them. These injuries include, but are not limited to: physical contact with other individuals; contact with the ground, surfaces, fixtures, and equipment; and strenuous exertions, quick movements, and changes of speed, which place stress on the cardiovascular, muscular, and skeletal systems. The specific injury risks vary from (1) minor injuries such as scratches, bruises and sprains, to (2) major injuries such as eye injury or loss of sight, joint injuries, back injuries, heart attacks, and concussions, to (3) catastrophic injuries including paralysis and death. I agree, on behalf of the Participant, to assume these risks. I understand that if I or the Participant have questions about possible hazards, it is our responsibilities to seek additional information from the dance staff prior to signing this Agreement. I also understand that, despite safety precautions, the Audition cannot guarantee that the Participant will not be injured 2. Waiver, Release and Hold Harmless: that in consideration for allowing the Participant to participate in the Audition, I agree not to sue and I hereby release, waive, discharge, hold harmless, indemnify, and defend the Workshop, its employees, staff, volunteers, agents, directors, affiliates, sponsors, representatives, and Mater Dei High School from any and all liability, losses, damages, claims, actions, and causes of action of every nature for any and all known or unknown, foreseen or unforeseen, bodily or personal injuries, property damage, or other loss relating in any way to the Participants’ involvement in the Audition. 3. Medical Release and Authorization: that I am unaware of any health issue or restriction that would affect the Participant’s involvement in the Audition. The Participant has had a recent medical evaluation and is fully fit for the Audition. In the event of an emergency, I authorize the dance staff to act for me in its best judgment in rendering any medical attention to the Participant. I further understand that I will be responsible for any medical expenses relating to the Participant’s involvement in the Audition.
