Survey of School Library Services and/or Partnerships During the Summer
The purpose of this form is to collect valuable information in regards to systems/schools whose school librarians or library programs provide summer services (circulation of books, open hours of operation, etc.) or partnerships (Bookmobile location/stop, summer reading challenges/activities) during the summer months.

This survey stems from a partnership at the Alabama State Department of Education (ALSDE) to provide valuable resources and information to parents and community members for the annual Summer Learning Challenge. The information provided by you in this survey will help the Summer Learning Challenge resources be more responsive to communities and parents by providing pertinent information as to summer reading supports and programs available from schools and school systems.

Also, this is an opportunity for school libraries to partner with public libraries to provide resources for students to continue reading during the summer months to combat the "summer slide."
