National High School Leadership Conference March 2021: Team Registration
Register your team in the slots below. Note that the first slot is the team captain- please provide an email address to contact. EACH TEAM IS TO FILL IN ONLY ONE FORM.

* A complete team is 5 members. Teams with less than five members will be paired accordingly but not guaranteed to be together. *

THIS FORM IS DUE MARCH 12th AT 11:59 PM PST. Teams that do not fill out this form risk the chance of NOT being allowed to participate in the Prepped Case Study Challenge.

Any teams or individuals that register after Friday, March 12th MUST email briana@illuminateuniverse.com stating the members of their teams (if any). We reserve full rights to assign them to a random team if they do not inform us of their team members’ names.
