MAAPP and APAB Acknowledgement

Below are a few major points of the Minor Athlete Abuse Prevention Policy (MAAPP) and Action Plan for Addressing Bullying (APAB) for FORM Swim Team. * One-on-one interactions between a minor athlete and Adult Participants must occur at an observable and interruptible distance.
* Meetings between athlete and Adult Participants must occur with another adult in an open door, observable location
* Another adult must be present during individual training sessions
* Parents must be included in any electronic communications between an Adult Participants and swimmer
* Legal guardians may request that their swimmer not be communication by electronic communication through written documentation
* Adult Participants may not send private messages to individual athletes by social media
* Adult Participants must not be in a vehicle alone with a swimmer
* Recording devices are prohibited in locker rooms for everyone
