Thank you for your interest in becoming a Working Group partner for Food Future WNY. We appreciate your passion and interest in strengthening the region's food system.

Attached to this form are the Operating Principles and the Stipend declaration form.

By acknowledging and agreeing to the Operating Principles Agreement, you are indicating your "official" participation of the Working Group(s) of your choosing. Only those who have returned the Operating Principles agreement will continue to receive correspondence and participate in group discussion and activities. As the working group format emphasizes decision and priority making through consensus, we encourage you to participate regularly so that you remain threaded into the evolving discussions and work.

Please read and acknowledge the Stipend declaration form, as well as noting whether you accept or decline the stipend.

If accepting a stipend, we ask that you also fill out a W-9 form, which is linked below. Please fill out the form, sign it, and return by email to Kristen Tim at kristen@sustainagriculturenys.org.
