Replacement Loyalty Card Request Form
Please use this form to request a new loyalty card if you have lost or misplaced your card. Your old account number will be void once the new card is issued, but all personal information on the account such as name and phone number will remain the same. Each customer is allowed up to 1 replacement card per year.
