COVID Student Exposure or Positive Test Report
This form is to report your student's COVID exposure or positive test results.  

TVCS families of enrolled students are encouraged to use the process below to report concerns about COVID-19 potential exposure or a positive test.  The reporting process is confidential and follows the guidelines and protocols developed by the California Department of Public Health and Riverside County Public Health.  

A TVCS representative will be in contact with you as soon as possible.

THE USE OF THIS PAGE IS INTENDED SOLELY FOR THE EXPLICIT PURPOSE OF REPORTING A FIRST-HAND/PERSONAL POTENTIAL EXPOSURE OR CONFIRMED COVID-19 TEST FROM A STUDENT'S PARENT/GUARDIAN.  ANONYMOUS AND/OR THIRD-PARTY SUBMISSIONS WILL NOT BE REVIEWED.
