By completing this form, you are giving consent to have the information you provide published in the Cushing PTO Family Directory.

This is strictly voluntary. If you choose not to complete this form, no information other than your student’s name will be included in the directory.

Important Notes:

* Enter only the contact information you wish to have published in this year’s directory.

* Fill out a form for each student you have attending Cushing.

* Parent/guardians residing in separate residences must provide information and consent to publish directory data independently, so each parent/guardian must fill out their own form to be included in the directory. To protect the privacy of all families, you may not submit information for the non-residing parent/guardian.
