We have received funding from Kingston council to support the Health and Well-being for members of the Sunray & Egmont Community over the next four months. We have devised this survey to find out about your views.

If you complete the survey, you can choose to be entered into a £50 prize draw.

Please invite all members of your family that are over 18 to complete the survey as the more information we receive the better we can tailor our support to meet your needs.

If you know a neighbour that doesn't have internet access, but you think may like to complete the survey, please email wellbeing@secra.org.uk and we can arrange for a printed copy.
