Rocky Creek Worship Service Registration Form

1. You must PRE-REGISTER in order to attend worship service.

2. There must be a registration for EACH PERSON that will attend.

3. You must answer the COVID Screening questions as a part of the registration process.

4. Masks are required at all times in the facility. Children 6 and under will not be required to wear a mask.

5. Ushers will guide you to your seat. DO NOT expect to seat yourself.

6. NO seating will take place after 10:15, in order to not disrupt the service.
