Once you submit this form, you must pay for your items in one of two ways:
1. Use the payment app Venmo (@AHS-FAD) last 4 digits of the phone are #3633 if needed
2. Mail a check for your total. Address is listed on the payment page at the end.

Orders will be processed once payment is confirmed and you will be contacted when your order is ready for pick-up.
