Before filling out this form, please go to the PHCSD.ORG web page, and click on the Guidance area on the top right. Once clicked, you will see the top right hand button shows as "SEAWAY TECH".  Click on that button.  You will see this form, the parent application form, and a link to 12 YOU-TUBE videos. You may watch all 12 videos, or go to the "hamburger" (3 horizontal little lines) on the top right of the video box, to get to the play list to choose only those videos which you want to see!
