I will participate in all 9 program day activities as outlined

I acknowledge that some outside time will be required for daytime commitments to successfully participate in our class project.

I acknowledge that this is a 2 year commitment and will serve as a Co-Chair planner for one day of the program the year following graduation.

I acknowledge there may be times where I will be closer than 6 feet to other participants. I must have a mask and hand sanitizer provided available at all times and use them when necessary if that is required by OHA at the time

I will have access to internet and a device with audio/video capability if needed for virtual components of the class program and will provide my own transportation when needed.

I understand the Tigard Chamber will run the program in compliance with OHA and CDC guidance and will hold them harmless from exposure to COVID-19.
