Weekly COVID Safety Checks (pooled testing) Opt-In Form
Weekly in-school COVID safety checks (formally known as pooled testing) will begin the week of 2/7/22. Test and Stay testing for in-school close contacts has been discontinued. These weekly checks are another tool for early identification of positive individuals who may not have symptoms.

If you have already completed a consent for your child to participate in test and stay, the same consent covers weekly COVID safety checks. Weekly COVID safety checks are performed by our testing nurses every week at your child's school and involve a simple self swab observed by the nurses. You will be notified only if your child tests positive for COVID 19. Your child may participate in both the weekly COVID safety checks (pool testing) and the at home antigen testing program. If your child tested positive for COVID within the last 90 days, they may begin the in-school testing on day 91. Individuals who are fully vaccinated are able to participate in the weekly in school testing.

BY completing this form you are opting your child into weekly in school COVID safety checks. PLEASE COMPLETE THIS FORM ONLY IF YOU WANT YOUR CHILD TO PARTICIPATE IN WEEKLY COVID SAFETY CHECKS. A new form needs to be completed for each child if you have more than one in the district.
