You will need to complete and submit this form to provide an official request of cancellation and begin the cancellation process. Please ensure that you select "Submit" at the end of this form.
Please also ensure your "Database Name" is spelled correctly in your request per the initial email you received.
If this form is not completed in its entirety, your request will not be delivered to our team and will be considered void.
Important Information:
Please be as open, honest and candid as possible in your responses and avoid non-specific answers such as "N/A" "None" or "No".
Fit Pro Tracker requires 30 days advanced notice to process your cancellation per our Terms of Use. Your request is placed in queue when this form is completed, and all cancellation requests will be handled in the order which they are received.

30 Day Notice Requirement:
In accordance with the Fit Pro Tracker Terms of Use, we require 30 days of advanced notice via completion of this form to cancel your database. Therefore, you may have a final bill due if you have requested cancellation within 30 days of your next billing cycle.
For example: if you are billed on the first of each month and request to cancel your account on June 16th, you will need to pay your July 1st bill. With this final payment you will be able to either
a) make the payment and close your account immediately, or
b) make the payment and maintain access through the paid period of your membership (using the above example, you would have access from July 1st to July 31st)
Details on your final payment will be provided by our team when this form is completed.
Refund Policy:
Per the Fit Pro Tracker Terms of Use, we cannot provide refunds.
Disabling Payment Processing Information:
Immediately after you have finished processing your members' payments in Fit Pro Tracker, make sure to disable billing for that client to ensure no transactions will process through Fit Pro Tracker. For added security, you may also place a few "X"s in the payment information to ensure no payments will process. This will ensure no one is charged after your transition date.
Exporting Information:
The Fit Pro Tracker Terms of Use outlines that you may export your information prior to closing your database. However, once your database has been closed you will no longer have access to that information. All information exported during the cancellation is free of charge, however any exported information following the closure of your account will result in a late export fee.
Standard Export Service (convenience fee of $199 required):
- Personal and Contact Information (All Prospects/Current and Past Clients)
- Membership History
- Billing History
- Payment Method Information
Contract/Waiver Export Service:
- Contact FPT Support for availability and pricing. Price depends on number and type of documents to be downloaded.
** Please Note: FPT does not charge any exit file fees on behalf of our payment processor. Additional fees may apply. For pricing information on an exit file from the payment processor, please contact FPT Support and we will request pricing for your exit file.
You will need to conduct manual exports of the following information:
- Payroll history
- Prospect/Client notes
We do not export your clients' payment information. To remain in compliance of both our agreements with our payment processors, as well as maintain PCI and Data Security standards, we are limited to serving purely as a liaison between Fit Pro Tracker clients and your payment processor. To request a secure transfer of this information, you will need to contact your payment processor directly and notify us of you desire to transfer those records and provide the new payment processor information.
Late Export Fee
If you do not request to export information in this form, and do not export information during the cancellation process, you will be subject to a late export fee. Regardless of the circumstances regarding your cancellation, we recommend exporting all information listed above. The Late Export Fee is $399.
Rescinding Your Cancellation Request
If you decide to continue with Fit Pro Tracker after completing this form, you will just need to provide written authorization to resume monthly billing and resolve any potential past due bills. Once we have resumed billing and successfully resolved past due bills (if any exist), your your cancellation request will be removed.
