Assalamu Alaykum,
The Sisters Committee at Masjid AlSalam has initiated this survey to determine the need for a program that connects the seniors within the Masjid AlSalam zip codes with people that live near them and are able to help them in times of great need, such as now due to the COVID-19 pandemic.  This public health emergency has caused widespread disruption to daily life which has affected the seniors much more acutely than the general population.

This form is to be used by both the seniors in the community as well as the volunteers that are able to support them by going out and bringing them any essentials, such as groceries, prescriptions, etc.  

Depending on feedback received from this survey, the Masjid Administration will work with the Sisters Committee to establish and deploy this program as soon as possible.
