ACS PTO Birthday Marquee Sign Up
Please use this form to indicate what date you would like your student's birthday posted.  Please submit one form per student.  Be mindful of weekends and school breaks, when selecting your date.  

ACS requires 48 hour notification to ensure the student's birthday is posted on the requested date.  Any submissions received with less than 48 hour notice will be posted on the marquee, but we cannot guarantee it will be posted on the requested date.

**Please note, if you select a date that falls on a weekend or a school break, your student's birthday will be posted on the Friday before the weekend or break.

Lastly, please be sure to submit payment to the ACS PTO.
