University at Buffalo HRSA Training Program Supplemental Application
This supplemental application is for candidates applying to the Health Resources and Services Administration (HRSA) training program for the Master of Public Health or Advanced Public Health Certificate in the School of Public Health and Health Professions at the University at Buffalo. After completing the traditional application required via SOPHAS for admittance to the UB School of Public Health and Health Professions, applicants must complete this additional supplemental application. After submitting the supplemental application, please email our team at SPHHP_HRSAtraining@buffalo.edu to verify all materials have been submitted and received.

Additional information:
1. This application is in additional to your application materials to your degree program and/or certificate program.
2. If selected for this program, continued funding is contingent upon the availability of funds and satisfactory performance in the program (which includes obtaining a B or better in all courses (B- does not count) as well as completing all extracurricular activities.  Additionally, completion of all required documentation is necessary to maintain your traineeship.
3. Tuition and fees are only covered for academic courses needed for the MPH or certificate program.
