Deadline to submit this form for ALL NCSU books/online access is Friday, August 26th at 5 PM. Any orders after this date will be purchased by the student.

Any books/online access codes not picked up by August 31st at 3 PM will be returned to the bookstore. Students will be responsible for purchasing any items not picked up by 8/31/2022, if needed for class.

You may submit this form more than once as your professors provide their required resources for each class.
