This is an online alternative for the green "Transcript Request Form" that I asked each senior to complete for every college application they have submitted.

All seniors that apply to college are required to submit either this or a green form to me so that I am aware that you applied on your end, so I can send your transcript, school report, teacher letters of recommendation, and more to the schools. If I do not receive a green form OR this virtual form, then your applications are INCOMPLETE!
