Following guidance from Alberta Health Services, people are required to screen for COVID-19 symptoms before coming to campus.

If you have been isolated or tested positive for COVID-19, you may be asked to consult a physician before participating in any activities at MRU.

The personal information that you provide in this form is collected under the authority of the Post-Secondary Learning Act and the Freedom of Information and Protection of Privacy (FOIP) Act - section 33(c). The information will be used to comply with Alberta Health Services' directives during the coronavirus pandemic. Collected personal information is protected from unauthorized access, collection, use and disclosure in accordance with the FOIP Act and can be reviewed upon request, subject to the provisions under the Act.
