Thank you for your interest in the Designing Healthier Places (DHP) program. The deadline for this application is October 15, 2021.
The 2021 program is limited to five community-based teams. Only teams in New Jersey communities may apply. To be eligible, you must:
*Identify the place where you want to focus your efforts. This could be a neighborhood, town, collection of towns, or a region within New Jersey.
*Have at least one of each of the following on your team -- Health professional (such as a doctor, nurse, medical institution administrator); local or county government official (elected or appointed); and a working artist (someone who gets at least some of their income from creating, producing , performing or exhibiting artistic products. In addition to painter, sculptors, writers and performers, this category also includes chefs, tattoo artists, game designers, and other types of creative professionals.)
*Identify at least one physical or behavioral health issue having a negative impact on your community (such as widespread obesity or drug abuse).
*Confirm that, if your team is selected and completes the program, it will use any seed money provided to begin implementing the strategies that it develops through the program.
This is a competitive program, and only five teams will be selected. Here are some tips to make your application stronger:
*Make your team more than three people, and have it be as representative of as many different interest groups in your community as possible.
*Ask your elected officials for a resolution supporting your application, or ask for letters of support from influential organizations in your community. You can send these letters separately to the Program Director, Leonardo Vazquez, at
